20. Shared Care Information

Recommended in both primary and secondary care Alternatives (often in specific conditions) in both primary and secondary care; Where a specialist input is needed (see introduction for definition) Prescribing principally within secondary care only

Shared Care information is available to enable GPs to take over prescribing from secondary care in certain areas.

An agreement to share the care is made between primary and secondary care using the 'Shared Care Agreement Letter' (see bottom of page for link).

The Shared care information will be reviewed at the appropriate time together with the appropriate chapter of the formulary.
